أثر التغطية الصحية للأمومة والطفولة على وفيات الأمهات والأطفال في الجزائر

Abstract

تهدف هذه الدراسة السوسيوديمغرافية الصحية إلى إظهار مدى تأثير التغطية الطبية و السريرية للأم و طفلها في سنته الأولى على معدلات وفيات الأمهات و الأطفال بين الولايات في الجزائر، و هذا باستعمال الكثافات الصحية ووفيات الأمهات الاستشفائية التي سُجلت في المؤسسات الصحية العمومية و المسجلة في وزارة الصحة و إصلاح المستشفيات، و وفيات الأطفال الرضع المنشورة من طرف ديوان الوطني للإحصائيات. من بين النتائج التي توصلت إليها الدراسة إلى أنه تختلف التغطية الصحية للأم و الطفل في الجزائر حسب الولايات و المناطق الجغرافية حيث أنها جيدة في الجنوب متوسطة في الشمال و ضعيفة إلى ضعيفة جدا في الهضاب العليا.كما بينت تلك النتائج أن معدلات وفيات الأمهات الاستشفائية لا تتبع نفس المسار حيث أنها مرتفعة في الجنوب متوسطة في الهضاب العليا ومنخفضة في الشمال مع بعض الاستثناءات حسب الولايات.أما معدل وفيات الأطفال الأقل من سنة فإنه متقارب بين المناطق مع ارتفاعه قليلا في الهضاب العليا، متوسط الارتفاع في الجنوب و منخفض في الشمال. وبسبب العدد الكبير من المتغيرات المستقلة، و الذي كان يجب تحليل علاقتها مجتمعة مع المتغير التابع و استبيان تأثيرها على وفيات الأمهات الاستشفائي من جهة و وفيات الأطفال الرضع من جهة أخرى، استعملنا طريقة الانحدار الخطي المتعدد المتغيرات الذي يعتبر طريقة إحصائية لتحليل المعطيات ملائمة لهذا النوع من المتغيرات الكمية المستمرة و الذي يقيم العلاقة بين متغيرات مستقلة كمية مستمرة و متغير تابع كمي مستمر. وقد بينت هذه الطريقة أي طريقة الانحدار الخطي المتعدد المتغيرات تأثير التغطية الصحية بالأطباء المختصين في طب النساء و التوليد و التغطية بالقابلات على مستوى وفيات الأمهات الاستشفائي بين الولايات، كما بيّن نفس التحليل تأثير التغطية بالأطباء العامين و أسّرة المستشفيات على مستوى وفيات الأطفال الأقل من سنة بين الولايات في الجزائر. This sociodemographic health study aims to show the impact of the medical and clinical coverage of the mother and her baby in the first year on maternal and child mortality rates among states in Algeria, using health densities and hospitalized maternal deaths that were registered in public health institutions and registered in the Ministry of Health and Hospital reform, infant mortality published by the National Bureau of Statistics. The study concluded that the health coverage of mother and child in Algeria varies according to the states and geographical regions, as it is good in the south, medium in the north, and weak to very weak in the high plateaus. In the south, it is medium in the high plateaus and low in the north with some exceptions according to the states. As for the mortality rate for children under one year of age, it is similar between regions, with a slight rise in the high plateaus, medium height in the south and low in the north. Due to the large number of independent variables, whose relationship with the dependent variable had to be studied at the same time, to show their impact on hospital maternal mortality on the one hand and infant mortality on the other hand, we used the method multivariate linear regression, which is a statistical method of data analysis adapted to this type of continuous quantitative variables, which evaluates the relationship between independent quantitative continuous variables and a continuous quantitative dependent variable. This method, i.e. multivariate linear regression, showed the effect of health coverage by obstetricians-gynecologists and coverage by midwives on the level of hospital maternal mortality between the willayas, and the same analysis showed the effect of coverage by general practitioners and hospital beds on the mortality rate of children under one year between willayas in Algeria.
