L’annonce Du Cancer: Quel Dispositif Et Quel Impact ?

Abstract

Nombreuses sont les personnes ayant eu un cancer qui considèrent qu’une des épreuves les plus marquantes qu’elles ont eu à affronter est celle de l’annonce, les mots prononcés par le médecin et ses gestes restant à jamais gravés dans leur mémoire. L'annonce de cette pathologie provoque des images de mort, de souffrance, de traitements longs et pénibles. Elle va, dans la plupart des cas, provoquer la « sidération » du patient qui va être dans l'impossibilité d'entendre ce que le médecin lui dit lors de la consultation. La personne malade passera ensuite par différentes phases de réactions psychologiques au cours desquelles son désir d'information et d'accompagnement évoluera. En réponse à cette demande et afin de permettre aux patients d’avoir les meilleures conditions d’annonce de leur pathologie, le dispositif d’annonce a été mis en place. Le dispositif d’annonce du cancer est par essence un processus d’objectivation : il consiste en la création, préalable à la rencontre d’un patient, d’un environnement pensé avant le trauma de l’annonce du cancer, et adaptatif à la spécificité de chaque situation. Cet environnement est composé de ceux qui, soit par leur métier soit par leur position d'aidant, apportent des soins et du soutien et qui s'investissent auprès d'un patient. Il y a donc, au cœur de ce dispositif une confiance du patient vers le soignant, mais aussi, du soignant envers le patient. Ce dispositif permet d’avoir une information mieux vécue, et comprise qui facilitera une meilleure adhésion du patient à la proposition de soin, et l’aidera à utiliser des stratégies d’adaptation à la maladie. En s’étayant sur des vignette clinique, l’objectif de ce travail est de souligner l’importance et l’utilité d’un dispositif d’annonce pour une meilleure prise en charge des patients atteints du cancer, en mettant l’accent sur la place du psychologue dans la consultation d’annonce.

Many people with cancer who consider that one of the most striking events that had to face is the announcement …the words spoken by the doctor and his gestures remain forever engraved in the memory .The announcement of this pathology induce pictures of death, suffering, long and painful treatments .it will, in most cases , cause the stunning , to be unable to hear what doctor say during the consultation , and will experience different phases of psychological reactions during which his desire for information and accompaniment will evolve. In response to this request and in order to allow the patient to have the best conditions of announcement of their pathology, the device of announcement has been put in place. This device is essentially a process of objectification: it consists in the creation, prior to the meeting of a patient, of an environment thought before the trauma of the announcement of the cancer, and adaptive to the specificity of each situation .This environment is composed of those who, through their job or caregiving position, provide car and support and who are involved with a patient. Therefore, this device requires mutual trust between patients and caregivers .It provides a better understood of information that will facilitate a better adhesion of the patient to the care proposal, and will help him/her to use strategies adaptation to the disease. Focusing on clinical vignette the aim of this work is to highlight the importance and usefulness of an announcement system for better management of cancer patients, and reveal the place of psychologist in the consultation of announcement.
