Dérive organisationnelle et sécurité des patients: les facteurs organisationnels des accidents en anesthésie

Abstract

Résumé: Les auteurs présentent des résultats tirés de la première étude d’une recherche en cours sur la migration des pratiques et la sécurité en anesthésie. L’accent est mis sur l’impact des transgressions organisationnelles sur l’activité des Auxiliaires Médicaux en Anesthésie Réanimation (AMAR) et leurs conséquences sur la sécurité des patients. L’analyse du travail prescrit de l’AMAR a mis en évidence le processus de passage d’une prescription floue à la normalisation d’une pratique illégale. L’observation de l’activité réelle et les entretiens menés avec les AMAR et d’autres personnels du bloc opératoire (médecins et chirurgiens), ont montré que l’AMAR, dans certaines situations, est amené à intervenir sans la présence du Médecin Anesthésiste Réanimateur (MAR). Dans de telles situations, la question de la sécurité du patient et même celle de l’AMAR, notamment par rapport à l’efficacité et aux conséquences de sa prise de décision, est posée. En effet, à partir de l’analyse de deux exemples, nous avons essayé de montrer comment les facteurs organisationnels contribuent à la survenue des accidents en anesthésie, lorsqu’une anesthésie est conduite sans la présence du MAR. ملخص: يعرض هذا المقال بعض نتائج اولية لبحث ما زال في طور اإلنجاز حول انحراف املمارسات واألمن في التخدير. ينصب التركيز على تأثير الخروقات التنظيمية على نشاط املساعدين الطبيين في التخدير واإلنعاش وعواقبه على أمن املرض ى. أبرز تحليل العمل املسطر لألعوان الطبيين في التخدير واإلنعاش عملية االنتقال من التسطير الغامض إلى قبول ممارسة غير قانونية بحيث تصبح عادية. أظهرت مالحظة النشاط الفعلي واملقابالت التي أجريت مع األعوان الطبيين في التخدير واإلنعاش في مختلف املؤسسات االستشفائيةوغيرهم من موظفي غرفة العمليات )األطباء والجراحين( أن األعوان الطبيين في التخدير واإلنعاش في بعض الحاالت يتدخلون دون حضور طبيب التخدير. في مثل هذه الحاالت، تطرح إشكاليةأمن املريض وحتى أمن العون الطبي فيالتخدير واإلنعاش خاصة فيما يتعلق بفعالية ونتائج اتخاذ قرارات هذا األخير. حيث قدمنا مثالين نوضح فيهما كيفية مساهمة العوامل التنظيميةفيوقوع حوادثالتخدير، عند إجراءالتخدير دون حضورالطبيباملخدر. Abstract: The authors present results from the first study of current research on the migration of practices and safety in anesthesia. The focus is on the impact of organizational transgressions on the activity of Medical Auxiliaries in Anesthesia Resuscitation (MAAR) and its consequences on patient safety. MAAR's prescribed work analysis highlighted the process of shifting from a fuzzy prescription to normalizing an illegal practice. The observation of the actual activity and the interviews conducted with the MAARs and other operating room staff (doctors and surgeons), showed that MAAR, in certain situations, has to intervene without the presence of the Anesthesiologist Resuscitator. In such situations, the safety of the patient and that of MAAR, especially with regard to the effectiveness and consequences of its decision-making, are in question. Indeed, from the analysis of two examples, we tried to show how the organizational factors contribute to the occurrence of accidents in anesthesia, when anesthesia is conducted without the presence of Anesthesiologist Resuscitator.
