نزوح الممارسات المهنية والأمن: دراسة تسيير المخاطر لدى الأعوان الطبيين في التخدير والإنعاش

Abstract

تتناول هذه الأطروحة موضوع الأمن وتسيير المخاطر في الأنساق المعقدة والديناميكية. حيث تمت الدراسة في ميدان التخدير كميدان طبي يدخل ضمن هذه الأنساق. فالتساؤل المطروح هو كيف تؤثر التجاوزات التنظيمية على نشاط وصحة الأعوان الطبيين في التخدير والإنعاش؟ وكيف يقوم أعوان التخدير بتسيير المخاطر المتعلقة بنشاطهم؟ للإجابة على هذه التساؤلات قمنا بإنجاز دراستين متكاملتين، أين استعملنا عدة طرق وتقنيات جمع وتحليل البيانات على عينة من أعوان التخدير مؤسستين استشفائيتين مختلفتين. لقد أظهرت نتائج الدراسة الأولى على أن الخروقات التنظيمية لها تأثير على نشاط أعوان التخدير من خلال التناقضات والمفارقات في التسطير وإنجاز مهام طبية معقدة في الواقع والتي كانت مصدرا للصعوبات والضغط لديهم. كما أطهرت الدراسة الثانية أن تسيير المخاطر عند أعوان التخدير يتم في مرحلتين. مرحلة تقييم والتنبؤ بالمخاطر قبل التكفل بالمريض بتقييم المخاطر والموارد الداخلية والخارجية ومرحلة مواجهة الوضعيات غير المنتظرة فرديا وجماعيا أثناء التكفل بالمريض في قاعة العمليات.

This thesis deals with the issue of security and risk management in complex systems. The research was carried out in the field of anesthesia as a medical field that falls within these systems. The question is, how do organizational violations affect the activity and health of medical personnel in anesthesia and resuscitation? How do anesthesia assistants manage the risksrelated to theiractivity? Twointegrated studiesw as carried out, whereweuseddiffrentmethods and techniques for collecting and analyzing data on a sample of anesthesia assistants fromtwo different hospital institutions. Weshowedthat organizationa lbreaches have an impact on the activity of anesthesia assistants through contradictions and paradoxes in underlining and accomplishing complex medical tasks in reality, which were a source of difficulties and pressure for them. Then,wealso showed thatrisk management for anesthesia assistants takes place in two stages. The stage of assessing and predictingrisksbe foretaking care of the patient, and the stage of facing unexpected situations while taking care of the patient in the operating room.
